🔍 November 2023: Inspection
During the course of this inspection, the inspector(s) made relevant observations,
reviewed records and conducted interviews, as applicable. There were no findings
of non-compliance.
🔍 August 2023: Inspection
This inspection report for Longfields Manor was carried out by Gurpreet Gill, Dee Colborne and Jessica Nguyen. The inspection included both complaint and critical incident system types.
- Infection Prevention and Control: There was a failure to implement a standard or protocol issued by the Director regarding infection prevention and control. Specifically, there was no signage indicating enhanced infection prevention and control measures for residents on additional precautions. This was remedied by posting the required signage.
- Plan of Care: There was a failure to follow the plan of care as specified, particularly in the application of an external device for a resident. This failure posed a greater risk of falls for the resident. Additionally, the provision of care outlined in the plans of care for three residents was not documented, particularly regarding scheduled baths or showers.
- Requirements Relating to Restraining by a Physical Device: The inspection found non-compliance with physician orders related to the application of a device for safety and fall prevention. The device was observed unfastened on multiple occasions, which increased the risk of falls for the resident.
🔍 June 2023: Inspection
This inspection report for Longfields Manor was led by Megan MacPhail and Mark McGill, focusing on a mix of complaints and critical incidents.
- Plan of Care: There were failures in ensuring clear directions for staff in the plan of care. This included discrepancies in a resident’s CPR plan, which was not coordinated with palliative care orders, and a lack of guidance for staff when a resident’s blood pressure did not meet the target. This lack of clarity could lead to confusion and inadequate care.
- One-to-One Monitoring: A resident required constant monitoring, but this was not adhered to when the assigned PSW went for lunch. Other staff did not provide consistent monitoring during this time, and the resident’s health condition changed.
- Falls Prevention and Management Program: The policy related to post-fall management and head injury routine was not followed properly. After an unwitnessed fall, the resident’s neurological vital signs were not monitored as required, potentially missing important health changes.
- Administration of Drugs: Medications were not administered as prescribed. In one case, medication was not given at the prescribed intervals without proper documentation or follow-up. In another case, there was a delay in initiating treatment due to a discussion about changing the medication form.

🔍 June 2021: Complaints Inspection
This inspection report for Longfields was led by Emily Prior. The report covers various aspects of the facility’s operations and care services.
The inspection focused on allegations of short staffing affecting care and monitoring of residents, food quality, maintenance services, and lack of supplies.
The report details a written notification of non-compliance regarding menu planning. Specifically, it was found that residents were not offered a between-meal beverage in the morning on two occasions, increasing their risk of dehydration.
Longfields Manor was requested to prepare a VPC to ensure compliance, particularly in offering between-meal beverages in the morning.

Specifically, the facility failed to ensure that temperatures in resident bedrooms, common areas, and cooling areas were measured and documented as required. This lack of temperature monitoring could compromise resident comfort and safety.
