These included issues related to the documentation for restraint use (Sub-Standard 16.3), policies and procedures for restraint use (Sub-Standard 16.1), medication management (Sub-Standard 12.1), infection prevention and control (IPC) policies (Sub-Standard 11.1), and managed risk agreements (Sub-Standard 10.1). Additional deficiencies were noted in the documentation and review of client care plans (Sub-Standards 1.8 and 1.4).

On August 11, 2021, Shepherd’s Care Millwoods underwent both a CMOH inspection under Public Health and an Accommodation Standard inspection. While the CMOH inspection found the facility compliant, the Accommodation Standard inspection revealed non-compliance related to menu substitutions and ensuring they are made from the same food group (Sub-Standard 14.2).

In November 2020, Shepherd’s Care Millwoods was subjected to multiple Public Health Quality Monitoring Visits and CMOH inspections. On November 26 and 25, the facility was found compliant with all standards. However, inspections conducted on November 13 and November 6 identified non-compliance in enhanced cleaning procedures (Sub-Standard 1.5), shared spaces (Sub-Standard 1.4), and the use of personal protective equipment (PPE) (Sub-Standard 1.2).

Inspections on October 30, October 27, and October 19 revealed non-compliance in areas such as enhanced cleaning (Sub-Standard 1.5), hand hygiene, PPE, nutrition and hydration, falls management, and staff contingency planning. Compliance was achieved in inspections conducted on October 16 and October 5.

In September 2020, Public Health Quality Monitoring Visits and a CMOH inspection were conducted at Shepherd’s Care Millwoods. The inspections on September 29 and September 28 identified non-compliance in enhanced cleaning (Sub-Standard 1.5), hand hygiene, PPE, nutrition and hydration, falls management, staff contingency planning, and shared spaces (Sub-Standard 1.4).

On May 21, 2020, a Public Health Quality Monitoring Visit was conducted, with the facility found compliant. However, a CMOH inspection on May 12, 2020, identified non-compliance in the implementation of COVID outbreak policies and procedures (Sub-Standard 1.3), shared spaces (Sub-Standard 1.4), and PPE (Sub-Standard 1.2).
