🔍  September 2023: Inspection

This inspection report for MacKenzie Place, conducted by Vernon Abellera, primarily addressed issues related to falls prevention and management, as well as resident care and support services.

  • Plan of Care: The licensee was found to be non-compliant with regulations, pertaining to the assessment by a Registered Dietitian (RD) to address risks to nutrition and hydration following a significant change in a resident’s health condition. This finding was based on a Critical Incident Report (CIR) related to a resident’s fall resulting in injury and subsequent hospitalization. Upon returning to the Long-Term Care Home (LTCH), the resident experienced a significant change in health condition, including nutritional issues and poor intake. Despite this, there was a failure to complete the required nutritional assessment by the RD, as per the home’s policy titled ‘Nutritional Care and Hydration’. This oversight by the RD and the lack of adherence to the home’s policy could have adversely impacted the resident’s nutrition and hydration status, contributing to their decline in health.

🔍  November 2022: Inspection

This inspection report, conducted at MacKenzie Place by Vernon Abellera and other inspectors, focused on a range of issues, including prevention of abuse and neglect, infection control, and falls prevention.

  • Plan of Care: The licensee failed to ensure a resident was assessed by a Registered Dietitian (RD) following significant changes in their health condition. This non-compliance was identified after a Critical Incident Report (CIR) was filed due to a resident’s fall resulting in injury and hospitalization. Upon their return, significant changes in their health, including nutritional issues, were noted. However, the RD did not complete the necessary nutritional assessment. This oversight could have negatively impacted the resident’s nutrition and hydration status, contributing to their decline in health.
  • Reporting Failure : There was a failure in reporting suspected abuse or neglect immediately to the Director. A resident reported verbal abuse to a Personal Support Worker (PSW), but the incident was not immediately reported to the home’s management.
  • Inadequate Reporting of Critical Incidents: The report of a resident’s fall and injury failed to include the name of the staff member who discovered the incident.
  • Unsafe Transferring and Positioning Technique: A resident experienced a fall due to improper positioning in their assistive device, indicating a failure to use safe positioning techniques.
  • Infection Prevention and Control Lapses: Non-compliance was noted in two areas: failure to ensure standards or protocols for infection prevention and control (IPAC) were followed, and failure to adhere to additional precautions for IPAC measure.
  • Minister’s Directives Not Followed: The home did not complete IPAC audits every two weeks as required by the Chief Medical Officer of Health (CMOH), increasing the risk of infectious diseases like COVID-19.
  • Multiple Instances of Neglect: There were several incidents of neglect by PSW staff, leading to resident harm or risk.
