The inspection report for AgeCare Westbury focused on multiple concerns, including a disease outbreak, prevention of abuse and neglect, and medication administration.

  • Infection Prevention and Control Program: The facility failed to monitor symptoms indicating the presence of infection in a resident as per the IPAC Standard. This oversight involved missing daily monitoring of critical symptoms like fever, coughs, and gastrointestinal issues, which could have delayed the detection and management of the resident’s condition.
  • Incident Notification: The facility did not notify a resident’s Substitute Decision-Maker (SDM) within the required 12-hour window following an incident of suspected abuse involving another resident. This lapse in communication prevented the SDM from being informed timely, potentially affecting the resident’s care plan development and implementation.

🔍  March 2023: Inspection

The inspection report at Chartwell Westbury Long Term Care Residence addressed intakes related to falls and alleged physical abuse towards a resident.

  • Plan of Care: An issue was identified where a resident’s plan of care was not updated following a reassessment by a Physiotherapist, who recommended a specified level of assistance for transfers. This oversight was rectified before the inspection concluded.
  • Reporting Certain Matters to Director: The facility failed to report an incident of alleged physical abuse of a resident immediately to the Director as required, potentially delaying the Director’s response to the incident.

🔍  January 2023: Inspection

The inspection report for Chartwell Westbury Long Term Care Residence focused on a medication incident and an injury of unknown cause.

  • Infection Prevention and Control: The licensee did not follow the Infection Prevention and Control (IPAC) Standard issued in April 2022, specifically, the failure to remove droplet contact precautions signage from the doors of two residents no longer in isolation. This was corrected on January 5, 2023, after the Registered Nurse was informed by the inspector.
  • Plan of Care: The facility did not implement a fall prevention intervention specified in a resident’s care plan. Observations confirmed the intervention was not provided, acknowledged by two Personal Support Workers (PSWs). This oversight posed a risk of injury to the resident.

In the first compliance order, the licensee was required to undertake specific actions, including re-training staff on Additional Precautions and maintaining written records of training and audits to ensure compliance with IPAC standards.

A second compliance order was issued due to non-compliance with the Minister’s Directive on COVID-19 response measures. It required review and re-training on universal masking and COVID-19 testing procedures, alongside conducting audits to ensure adherence to proper practices.

🔍  September 2022: Inspection

The inspection for Chartwell Westbury Long Term Care Residence served as a follow-up to previous compliance orders and involved several intakes related to falls resulting in fractures and issues concerning the Duty to Protect.

  • IPAC Standards: The main finding of non-compliance related to the Infection Prevention and Control Program. Specifically, the home failed to implement standards or protocols issued by the Director concerning infection prevention and control, notably in the provision and accessibility of 70-90% Alcohol-Based Hand Rub (ABHR). During the inspection, 37 expired hand sanitizers were found in use throughout the facility, which the IPAC Lead acknowledged should have been replaced. The use of expired hand sanitizers poses a risk of infection transmission due to potential reduced effectiveness against pathogens.

A previously issued Compliance Order was found to be in compliance.

The inspection report for Chartwell Westbury Long Term Care Residence, conducted by Noreen Frederick and Christine Francis, focuses on complaints related to various aspects of resident care, including skin and wound care, residents’ rights, and duty to protect.

  • Wall Repairs: Instances of non-compliance were found but had been remedied by the licensee prior to the conclusion of the inspection. This included repairs to holes in the wall of multiple rooms that were promptly addressed upon notification.
  • Plan of Care: The licensee failed to ensure adequate collaboration between Personal Support Workers (PSWs) and Registered Practical Nurses (RPNs) regarding the assessment of a resident experiencing pain, leading to missed opportunities for pain management.

A compliance order was issued due to a failure to protect a resident who exhibited sudden and unusual symptoms. Delays in notifying the physician and Substitute Decision Maker (SDM) resulted in the resident missing the timely treatment they required.
