🔍 September 2023: Inspection
The inspection for McCall Centre Long Term Care Interim Unit focused on various aspects of the facility’s operations. The inspection was completed by inspectors Noreen Frederick and Ryan Randhawa.
- Plan of Care: Care for residents #009 and #007 was not provided as specified in their plans. This included the failure to offer nutritional supplements and assistive aids, risking inadequate nutrition and compromised skin integrity.
- Documentation Issues: Incorrect documentation for the provision of care for residents #007, #005, and #008. This error posed minimal risk but indicated a lack of precision in care recording.
- Resident and Family/Caregiver Experience Survey: The facility did not seek advice from the Residents’ Council for conducting and acting on survey results, potentially omitting valuable feedback.
- Family Council Meetings: Semi-annual meetings to inform about the establishment of a Family Council were not convened, missing an opportunity for family involvement.
- Unsafe or Inaccessible Doors: A laundry chute door was left open and unlocked, presenting a safety hazard.
- Communication System Accessibility: A call bell was not accessible to a resident, risking delayed assistance during emergencies.
- Air Temperature Monitoring: Failures in measuring and documenting air temperatures in resident bedrooms, common areas, and cooling areas, creating risks for heat-related illnesses.
- Menu Planning: The facility did not offer snack choices and failed to review menu cycles with the Residents’ Council, compromising dietary variety and preferences.
- Food Service Temperature: Cold food entrée served at unsafe temperatures, posing health risks.
- IPAC Lead Work Hours: The IPAC lead did not work the required hours, impacting the effectiveness of infection control.
- Medication Management System Evaluation: Inadequate quarterly evaluation due to the absence of required team members.
- Continuous Quality Improvement Committee Composition: The CQI committee lacked essential members, potentially affecting the quality of improvement initiatives.
🔍 October 2022: Inspection
The inspection for McCall Centre Long Term Care Interim Unit was overseen by Inspector Wing-Yee Sun. This Service Area Office Initiated (SAOI) inspection focused mainly on Infection Prevention and Control (IPAC) practices.
- Hand Hygiene Program: The licensee was non-compliant with the IPAC Standard regarding the hand hygiene program. Specifically, expired Alcohol-Based Hand Rub (ABHR) was found and used by a staff member, which was not effective. This non-compliance was remedied on October 18, 2022.
- Posting of Inspection Report: The licensee incorrectly posted a Licensee Copy of an inspection report on the bulletin board, which could contain private information. This was corrected on October 19, 2022, by replacing it with the Public Copy.
- Environmental Cleaning: The licensee failed to adhere to the IPAC Standard for environmental cleaning, specifically not cleaning frequently touched surfaces in resident rooms and hallways as required. This included items like bed rails, light cords, call bells, and handrails. The risk was an increased transmission of infection.
- Surgical Mask Application: A Registered Practical Nurse (RPN) was observed not wearing their surgical mask properly, which increased the risk of infection transmission. The facility’s policy required masks to cover both mouth and nose.

During the course of this inspection, Non-Compliances were not issued.
🔍 February 2021: Other Inspection
The inspection for McCall Centre Long Term Care Interim Unit focused on surveillance testing concerns, specifically related to COVID-19. The inspection was completed by Nital Sheth.
- Minister’s Directive: The main issue was the failure to comply with the Minister’s Directive dated January 8, 2021, regarding COVID-19 long-term care home surveillance testing and access. This directive required staff to undergo Polymerase Chain Reaction (PCR) testing every seven days, as the McCall Centre was located in a public health unit region classified as a grey zone. However, it was found that several staff members (PSW #100, RPN #101, PSW #103) worked multiple days without having taken a PCR test within the past 7 days, violating the directive.
As a result of this non-compliance, a Compliance Order was served to the licensee, Trillium Health Partners. This order required the licensee to ensure that all staff in the home were tested for COVID-19 according to the most current Minister’s Directive.
