🔍  September 2023: Inspection

The Critical Incident inspection at Extendicare Medex, conducted by inspectors Maryse Lapensee and Kelly Boisclair-Buffam, addressed intakes related to a resident’s fall resulting in injury and alleged resident-to-resident physical abuse.

  • Infection Prevention and Control: The licensee failed to follow the IPAC standard related to additional precautions, including proper use of Personal Protective Equipment (PPE), appropriate selection, application, removal, disposal, and hand hygiene program. Specifically, during the afternoon snack pass in September 2023, ten residents were not offered assistance with hand hygiene, and a Personal Support Worker (PSW) did not perform self hand hygiene between serving residents. This non-compliance increases the risk of disease transmission among residents and staff.
  • Failure to Follow Contact Precautions: Observations revealed that two PSWs did not wear isolation gowns or perform hand hygiene when providing care to residents on contact precautions, contrary to the IPAC Standard and home policies.
  • Reporting Critical Incidents: The licensee did not contact the hospital within three calendar days after a resident was taken to the hospital following a fall in July 2023. This delay in communication hindered the timely submission of a Critical Incident Report (CIR) and assessment of the resident’s health condition post-hospitalization.

🔍  August 2023: Inspection

During the course of this inspection, the inspector(s) made relevant observations, reviewed records and conducted interviews, as applicable. There were no findings of non-compliance.

🔍  June 2023: Inspection

The Critical Incident inspection at Extendicare Medex, conducted by inspectors Karen Buness, Margaret Beamish, and Shevon Thompson, addressed intakes related to a resident’s fall resulting in a significant change in health status and a hypoglycemia event.

  • Falls Prevention and Management: The licensee failed to comply with its Falls Prevention and Management Program policy. Specifically, after a resident experienced a fall, vital signs were not monitored consistently as per the policy, which requires monitoring every hour for four hours, then every eight hours for 72 hours post-fall. This lapse was confirmed through interviews and a review of the resident’s Electronic Health Record.
  • Infection Prevention and Control: The licensee failed to ensure that staff followed the infection prevention and control standard related to the hand hygiene program. Observations revealed that during meal times, the majority of residents did not receive physical assistance or prompting to perform hand hygiene before meals. This was confirmed through interviews and aligns with the IPAC Standard.

🔍  April 2023: Inspection

The inspection at Extendicare Medex highlighted two critical areas of non-compliance.

  • Falls Prevention and Management: After a resident experienced a fall, the policy for monitoring vital signs was not followed as required. The policy stipulates that vital signs should be monitored every hour for the first four hours following a fall, and then every eight hours for the next 72 hours. This lapse in protocol was confirmed through interviews with staff and a review of the resident’s Electronic Health Record.
  • Infection Prevention and Control: Staff did not adhere to the standards of infection prevention and control, specifically regarding hand hygiene. During meal times, it was observed that most residents were not assisted or prompted to perform hand hygiene before their meals. This failure was established through observations during meal services and corroborated by staff interviews.

🔍  November 2022: Inspection

During the course of this inspection, the inspector(s) made relevant observations, reviewed records and conducted interviews, as applicable. There were no findings of non-compliance.

🔍  August 2022: Inspection

During an inspection at Extendicare Medex, the inspection team identified a significant issue regarding the non-compliance of safe transferring techniques, specifically involving a resident and two Personal Support Workers (PSW).

  • ⚠️ Safe Transferring Technique: The facility failed to ensure that safe transferring techniques were used when assisting a resident. A resident was being assisted by two PSWs using a mechanical lift. During the transfer, the sling became unhooked, resulting in the resident falling and sustaining an injury. An investigation conducted by the facility revealed that the supports for the sling were not properly in place during the transfer, contributing to the incident. The resident involved in the incident sustained an injury due to the fall. This injury necessitated a transfer to the hospital for further treatment.

🔍  August 2022: Critical Incident Inspection

The inspection for Extendicare Medex highlighted critical incidents and compliance issues.

  • ⚠️ Falls Prevention and Management: A resident experienced multiple falls, one of which resulted in serious injury and eventual death. The fall risk assessment and post-fall assessment were not adequately conducted as per the policy.
  • Reporting: There was a failure to report an incident where a resident caused another resident to fall, resulting in minor injuries. The incident was not reported as required to the Ministry of Long-Term Care. The Director of Care (DOC) indicated that the nurse should have reported the incident to the Ministry and the home’s manager. However, the reporting was delayed.

The inspector issued a compliance order to ensure adherence to the falls prevention and management policies. The licensee was directed to perform weekly audits to ensure compliance with falls prevention policies, including fall risk assessments and post-fall assessments.
