Introduction

Evergreen Care is located at 1550 Oxford Street in White Rock, a neighborhood known for its seaside charm and scenic waterfront. The facility is situated near several parks, including the expansive Centennial Park, which offers walking trails and recreational areas for residents and their visitors.

The facility opened in 1996 and is operated by the Baptist Housing Society, a not-for-profit organization.

Evergreen Care is not co-located with an assisted living residence.

The average wait time for admission is approximately two months.

Contracted Services

The facility does not use contracted services for professional nursing or non-regulated services. Housekeeping services are contracted, while building maintenance is managed internally.

Food services are provided by the operator, and all meals are prepared and cooked on-site. This arrangement allows for more direct control over the quality and nutritional content of meals served to residents.

Evergreen Care offers a total of 252 beds, with 249 of them publicly funded. The remaining three beds are privately funded.

Monthly services for cable, telephone, and internet are not included in the resident fees, requiring residents to arrange these services privately.

Licensing & Incidents

Evergreen Care reported no licensing complaints during the review period, and thus, no substantiated licensing complaints.

In terms of incidents, there were 81 expected deaths, 33 cases of unexpected illness, 31 falls with injury or adverse events, and two other injuries. One case each of aggressive behavior, abuse or neglect, and disease outbreak were also recorded. These figures may suggest ongoing needs for monitoring and prevention of such events within the facility.

Evergreen Care underwent one inspection, which resulted in five licensing infractions. The infractions included one for physical facility, two for staffing, and one for care and supervision. Families and caregivers can maintain open communication with staff and participate in family council meetings to monitor these areas and ensure that care standards are consistently met.

Resident Profile & Quality Indicators

The average age of residents at Evergreen Care is 85 years, with 63% of the population being female. Approximately 60% of the residents are aged 85 or older, and the average length of stay is about two years.

Ten percent of residents are diagnosed with depression, and 46% are receiving medication for depression. Thirty-five percent of residents are taking antipsychotic drugs without a diagnosis of psychosis, and 9% display physically abusive behavior. These figures may indicate a focus on managing mental health and behavioral challenges within the resident population.

Sixty-one percent of residents have dementia, and 27% have severe cognitive impairment, including severe dementia. This highlights the facility’s need to cater to the cognitive care requirements of many of its residents.

Forty-eight percent of residents take nine or more medications, reflecting a high level of healthcare needs within the facility. Additionally, 1% of residents had four or more emergency room visits.

Forty-seven percent of residents use a wheelchair, and 21% are dependent on assistance for activities of daily living. The facility reported that 1% of residents experienced worsened pressure ulcers, which underscores the importance of continued attention to mobility and skin care needs.

Sixteen percent of residents experienced falls in the last 30 days, and 9% are under daily physical restraints. These figures highlight the ongoing efforts in fall prevention and the monitoring of restraint use.

Vaccination Coverage

At Evergreen, 86% of residents are vaccinated for COVID-19, and 83% have received influenza vaccinations.

Among healthcare workers, 12% are vaccinated for influenza.
