🔍  January 2023: Inspection

This inspection report for Baywoods Place was conducted by Erin Denton-O’Neill, Jonathan Conti, and Ruzica Subotic-Howell.

  • Infection Prevention and Control Lead: The facility failed to have a dedicated IPAC lead in the home, as required. The designated IPAC Lead was on leave for three months, and their responsibilities were divided among several roles, including the Executive Director and Director of Care. This absence posed a risk to the effectiveness of the IPAC program.
  • Critical Incident Reporting: There was a delay in reporting a COVID-19 outbreak to the Director. The outbreak was recognized on November 29, 2022, but the Director was only informed on December 1, 2022, which could have implications for timely response and containment measures.
  • Directives by Minister: A staff member and a visitor were observed not adhering to universal masking guidelines, posing an increased infection risk to residents. Additionally, the facility failed to ensure a visitor completed COVID-19 asymptomatic screening testing before entering, increasing infection risk.
  • IPAC Program Standards: The hand sanitizer wipes used in the facility did not meet the required 70-90% alcohol content, compromising hand hygiene effectiveness. Moreover, a staff member incorrectly removed Personal Protective Equipment (PPE) upon exiting a resident’s room under additional precautions, increasing the risk of infection spread.

🔍  November 2022: Inspection

The inspection for Baywoods Place focused on various aspects of resident care and facility operations. The inspection team included Karlee Zwierschke and Daria Trzos.

  • Infection Prevention and Control: A Personal Support Worker (PSW) entered a room without appropriate PPE as indicated for droplet/contact precautions. The issue was resolved with the removal of unnecessary precaution signage and updating the care plan.
  • Plan of Care: The facility failed to adhere to a resident’s care plan that required two staff members for care. A PSW provided care alone, contrary to the plan. The Executive Director confirmed this lapse in following the specified care plan.
  • Substitute Decision-Maker: The resident’s Substitute Decision-Maker (SDM) was not fully involved in the care plan, particularly regarding testing for an infection. The SDM was unaware of test results and subsequent actions, indicating a lack of communication and involvement.
  • Behavioural Issues: The care plan for a resident with behavioral issues related to their diagnosis was not effectively reassessed. The facility did not adequately address long wait times for responding to the resident’s call bell, particularly during evening shifts. The Executive Director acknowledged the issue and planned to reassess the care plan.

🔍  December 2021: Critical Incident Inspection

The inspection for Baywoods Place focused on falls prevention, infection prevention and control, and pain management. The inspection team included Jobby James and Yuliya Fedotova.

  • Plan of Care: The plan of care lacked clear directions for staff, resulting in a resident not regularly receiving assistance with a specialized device, causing discomfort and risk of injury.
  • Pain Policies and Procedures: The home failed to comply with pain assessment and management policies. In particular, staff did not use the home’s assessment tool to monitor pain for residents #001 and #008 after administering pain medication, putting them at risk of inadequate pain management.
  • Infection Prevention and Control Program: The home did not ensure all staff participated in the IPAC program. Key issues included the lack of immediate access to appropriate PPE, such as eyewear for contact/droplet precautions, and staff not using eye protection while delivering snacks to a resident requiring contact/droplet precautions.

🔍  April 2021: Complaints Inspection

The inspection for Baywoods Place was conducted by Dianne Barsevich.

  • Bed Refusal Issue: The home did not approve an applicant’s admission following a review of assessments and information unless it lacked the necessary physical facilities or nursing expertise to meet the applicant’s care requirements. The bed refusal reason provided by BayWoods Place was deemed unacceptable grounds for withholding admission.
