The audit also found non-compliance in palliative and end-of-life care documentation (Sub-Standard 5.3), documenting and communicating changes in care plans (Sub-Standard 1.9), client nutrition and hydration assessment (Sub-Standard 13.1), medication management policies (Sub-Standard 12.1), and ensuring the provision of or referral to assistive equipment (Sub-Standard 6.1).

Additionally, deficiencies were noted in documenting medical status assessments (Sub-Standard 1.4), restraint use (Sub-Standards 16.3, 16.2, and 16.1), and the overall management of client care plans (Sub-Standards 1.3, 1.8, and 1.2).

On February 7, 2023, an inspection under the Accommodation Standard was conducted at Allen Gray Continuing Care Centre. The facility was found to be compliant with the standards being assessed during this inspection.

On April 11, 2022, a CMOH inspection under Public Health took place at Allen Gray Continuing Care Centre. The inspection identified non-compliance in the implementation of COVID outbreak policies and procedures (Sub-Standard 1.3).

On April 1, 2020, a consultation under the Accommodation Standard was conducted at Allen Gray Continuing Care Centre. The facility was found to be compliant with all relevant standards during this consultation.

On October 29, 2019, a CCHSS audit at Allen Gray Continuing Care Centre revealed multiple areas of non-compliance. These included issues with incident reporting (Sub-Standard 19.2), emergency preparedness (Sub-Standard 18.1), and various aspects of restraint use, including policies, documentation, and interdisciplinary review (Sub-Standards 16.1, 16.3, 16.4, 16.5, and 16.6).

The audit also identified deficiencies in policies for safe water temperatures (Sub-Standard 15.1), medication management (Sub-Standard 12.1), infection prevention and control (Sub-Standard 11.1), managed risk agreements (Sub-Standard 10.1), staff training (Sub-Standards 9.2 and 9.3), maintenance of equipment (Sub-Standard 6.3), palliative and end-of-life care documentation (Sub-Standard 5.3), and the review and documentation of client care plans (Sub-Standards 1.2, 1.4, and 1.8).

🔍  June 2019

On June 10, 2019, inspections were conducted under both the RFCA and Accommodation Standard at Allen Gray Continuing Care Centre. The RFCA inspection identified non-compliance in the requirement for meetings every six months to inform residents (Sub-Standard 2(3)d). The Accommodation Standard inspection found non-compliance in maintaining the accommodation in a safe condition (Sub-Standard 2.1).

On June 14, 2018, inspections under both the RFCA and Accommodation Standard were conducted at Allen Gray Continuing Care Centre. The RFCA inspection identified non-compliance in establishing a written process for addressing concerns (Sub-Standard 5.f), providing copies of the process to address requests and concerns (Sub-Standard 5.g), and designating a facility representative (Sub-Standard 5.a). The Accommodation Standard inspection found the facility to be compliant with the standards under review.
